Community Staff Nurse
Communication Skills To provide up to date information for families in conjunction with the multi-disciplinary professional team. To use the lone worker device to maintain effective communication with the wider team and to enhance personal safety. To undertake liaison with other staff and outside agencies to ensure continuity of care. Demonstrate effective verbal and written communication skills, both detailed and legible. Ensure patient and family confidentiality at all times in line with NMC guidelines. To participate in an effective communication system, which ensures that a team approach to family centred care is encouraged and adopted. Analytical Skills Maintain training and competency for all medical devices used within the community settings. Be aware of calibration of allocated medical devices. Planning and Organising Skills To assess, plan, implement and evaluate family centred care without direct supervision that is evidence based/ best practice. To maintain high standards of nursing care and to promote and maintain an environment suitable for the care of children within the community. To maintain a role as a primary nurse at all times through admission to discharge addressing care requirements sensitive to the individual needs of the patient and family. Maintaining patient records and confidentiality. To contribute to and participate in clinical audit and research programmes. Ensure the privacy and dignity of patients and their families is maintained and religious and cultural beliefs are respected at all times. Responsibility for Patient/Client Care To act as an effective role model to all staff, student nurses, children and their families and to promote and always maintain a professional appearance and manner in accordance with NMC guidelines and Trust Uniform Policy. To contribute to quality initiatives and the be aware of Quality Team information hub. To be familiar with and adhere to the NMC code of professional conduct, developing own role within professional guidelines and accountability and taking personal responsibility for own professional growth and keep up to date with professional development and research including attending both mandatory training and non-mandatory training opportunities. To ensure that all the Trusts policies and processes are adhered to. Responsibility for Policy/Service Development Maintaining patient records are legible and accurate at all times To participate in incident reporting as required. Promote and maintain a safe and comfortable therapeutic environment for children young people and their families. To ensure that all Trust policies and procedures are adhered to. Maintain readiness of all equipment ensuring that it is decontaminated after use and report if equipment is broken or faulty. Contribute to evidence based nursing practice policies and procedures. Responsibility for Financial & Physical Resources Contribute to the effective use of levels of PPE and Consumables to reduce waste and support costs. Physical Skills To be attentive and demonstrate high levels of concentration across the working day. To be able to drive and have use of a car. To have a full driving licence Responsibility for Research and Development To support and teach new members of staff and work alongside pre-registration students. To participate in professional development programmes, attend mandatory training days and keeping updated with new research findings, PDPs and role expansion. Acknowledges any limitations of competence and comply with all professional and trust rules, codes, policies and procedures To develop nursing practice within the clinical area to the highest possible standard.
